I actually think future Sam is the big surprise/shock/twist of the episode.
He's going to be THE reason Dean and Sam get back together. Oh and I'm taking a guess here... Present Dean:

No wait! I just watched it again and seeing the pics of present and future Dean, I now realize what it shows!
It's Future!Evil!Sam stepping on the neck of Bitter!Unhappy!Future!Dean and Present!Dean walks up behind them.
